We are looking for a Community Habilitation Staff to join our team and provide support to individuals with disabilities. The successful candidate will be responsible for developing individualized plans, providing direct services, and assisting with activities of daily living. This position requires excellent communication and interpersonal skills and the ability to work independently and collaboratively with other professionals. The Community Habilitation Staff must be patient, understanding, and passionate about helping individuals reach their full potential.

Responsibilities:

  • Develop and implement individualized plans for each client.
  • Provide direct care and support to clients.
  • Monitor client progress and adjust plans as needed.
  • Provide training in daily living skills, social skills, and other areas of need.
  • Assist clients in accessing community resources.
  • Collaborate with other professionals to ensure the best outcomes for clients.
  • Maintain accurate records of client progress and activities.

Job Type: Full-time

Pay: $25.00 per hour

Schedule:

  • 10 hour shift
  • 12 hour shift
  • 4 hour shift
  • 8 hour shift
  • Day shift
  • Monday to Friday

Experience:

Caregiving (Required)

License/Certification:

Driver’s License (Preferred)

CPR Certification (Preferred)
